Texas Tech General Information Sheet: Important data (e.g., student-athlete contact information, parents' information, insurance information, vehicle information, academic policies and guidelines) to be kept in student-athlete file.
Texas Tech Historical Form: Important data (e.g., eligibility, financial aid, equipment agreement, comp admissions agreement, gambling agreement, extra benefits, and housing) to be kept in student-athlete file.
Texas Tech Employment Form: Must be complete and on file prior to the first competition date. This form aids in the monitoring of student-athlete employment.
NCAA Student-Athlete Statement: Must be complete and on file prior to the first date of competition in your sport.
Part 1: The student-athlete affirms that they meet the NCAA regulations regarding eligibility, recruitment, financial aid, amateur status and involvement in organized gambling;
Part 2: The student-athlete signs the Buckley Amendment Consent form which certifies that they agree to disclose educational records.
Part 3: For incoming freshmen, the student-athlete affirms the validation of the ACT or SAT score.
Part 4: The student-athlete affirms that they have not tested positive for banned substances by a non-NCAA athletics organization and agree to drug testing by the NCAA if they have.
NCAA Drug Testing Consent Form: Must be complete and on file prior to the first day of practice in your sport. The student-athlete signs that they are aware of the NCAA drug-testing program and agrees to NCAA drug testing.
Big 12 Drug Testing Consent Form: Must be complete and on file prior to the first day of practice in your sport. The student-athlete signs that they are aware of the Big 12 drug-testing program and agrees to Big 12 drug testing.
Texas Tech NCAA Banned Drug Classes: Must be signed and on file. The student-athlete affirms that they have received a copy of the NCAA Banned Drug Classes.
FERPA Form: Must be signed and on file. The student-athlete must sign a consent for the disclosure of personally identifiable personal medical records.
International Eligibility Form: For new international student-athletes only. Must be complete and on file with the certification officer prior to the first competition date.
