Texas Tech Mid-Year Scholarship
Student-Athlete Paperwork Process

Please read carefully the steps listed below. You must complete all steps BEFORE you can practice.

Step 1: Please complete the following for Academic Services:

  • Academic Policies and Guidelines Agreement Form/Buckley Amendment on General Information Sheet (see Student-Athlete Handbook which will be provided upon arrival to campus).

    Step 2: Please complete the following for the Training Room:

  • Insurance and Medical Forms Signature Sheet.
  • Student Registration Form.
  • Medical Insurance Information Form.
  • Copy of Health Insurance Card (front and back).
  • Sports Physical (you will need to see the trainer for your sport to schedule - see below).

    Step 3: Please complete the following for the Compliance Office:

  • General Information Sheet/Vehicle Information.
  • First-Time/Returning Historical Form.
  • Employment Form.
  • NCAA Student-Athlete Statement.
  • FERPA Form.
  • NCAA Drug Testing Consent Form.
  • NCAA Banned Drug Classes Form.
  • Big 12 Drug Form.

    *Read the Compliance section in the Student-Athlete Handbook-to be provided upon arrival to campus*


    When steps 1-3 are complete, you need to see the following people in the order they are listed so they can review your paperwork and sign your clearance sheet.


    1. Matt Pickering (Athletic Academic Services Office - located in the Athletic Dining Hall ~ 742-0150). Matt will give you a Student-Athlete Handbook and review the Academic Policies and Procedures Statement.
    2. Trainer - The trainer will check your paperwork, physical and proof of insurance (see below).

  • Jon Murray - Men's Basketball, Golf, Track, Softball & Volleyball (United Spirit Arena Training Room ~ 742-7551)
  • Imelda Garcia - Women's Basketball & Tennis (ATC Training Room - across from Jones SBC Stadium ~ 742-1946)
  • David Steadman - Baseball & Soccer (ATC Training Room - across from Jones SBC Stadium ~ 742-1946)
  • Mark Chisum - Football (Jones SBC Stadium Training Room downstairs ~ 742-3355 x 295)
    3. Lori Smith (Athletic Compliance Office Room 108 - located in the South Stadium Offices - first floor beneath football ~ 742-3355 x 283). Lori will check all compliance related paperwork and clear you to practice ONLY. Lori will give your coach a copy of your completed Clearance Form. You cannot practice until coach has received this copy.
    4. Your coach will notify you once you have been certified for athletic competition.

    REMEMBER! YOU MUST COMPLETE THE ABOVE STEPS BEFORE YOU CAN PRACTICE!

    Texas Tech Walk-On Paperwork Process Please read carefully the steps listed below. You must complete all steps BEFORE you can practice.

    Step 1: Coach initiates walk-on process by providing you with a packet of paperwork.

    Step 2: Schedule a sports physical at Thompson Hall (743-2848). This should be done immediately. You must bring a copy of your physical to the trainer for your sport (see below).

    Step 3: If you have not already registered with the clearinghouse please do so immediately. The process to apply online is enclosed. You must have a copy of your 48-C (the clearinghouse will mail to you) or a copy of the clearinghouse application (print from web page) confirming your registration to the clearinghouse.

  • Copy of NCAA Clearinghouse Application (print from web page) OR Copy of 48-C

    Step 4: Please complete the following for the Training Room:

  • Sports Physical
  • Insurance and Medical Forms Signature Sheet
  • Student Registration Form
  • Medical Insurance Information Form
  • Copy of Health Insurance Card (front/back) ~ You must have medical insurance to walk-on.

    Step 5: Please complete the following for the Compliance Office:

    If steps 1-5 above are completed prior to September 9th, then report to the Athletic Dining Hall on Tuesday, September 9th between 3-4:30 p.m. with all required information. If you complete steps 1-5 after September 9th, you need to see the following people in the order they are listed so they can review your paperwork and sign your clearance sheet.

    4. Lori Smith (Athletic Compliance Office - located in the South Stadium Offices Room 108 - first floor beneath football ~ 742-3355 x 283). Lori will check all compliance related paperwork and clear you to practice ONLY.
    5. Lori will give you a copy of your completed Clearance Form. You cannot practice until you have handed this copy to your coach. The completed Clearance Form will be filed in the Athletic Compliance Office.
    6. Your coach will notify you once you have been certified for athletic competition.
